This week is National Advance Care Planning Week, a reminder to all Australians to have a conversation with their loved ones about what ‘living well’ means to them – and who they would want to speak for them if they weren’t able to speak for themselves.
Launched last year, the initiative encourages people, regardless of their age or health status, to make their future wishes known.
This can be done by creating an Advance Care Directive – a document that sets out your wishes around lifestyle, health and end-of-life decisions – and appointing a substitute decision maker (Power of Attorney).
