The Advance Care Planning Advisory Service provides free personalised advice on a range of issues including:
- How to start the conversation with family and friends;
- Choosing someone to make decisions for you if you are too sick to speak for yourself;
- Making your values and preferences known;
- Accessing the right forms for your state or territory; and
- Completing an Advance Care Directive.
You can’t control everything about getting older. But having an advanced care plan in place can help you and your family prepare for a time when you may not be able to make your own health care decisions. It can also offer peace of mind knowing your wishes will be respected.
